NHF Annual Meeting HSC Scholarship Application 

(Due to HSC Office not later than May 15th of each calendar Year)

 

REMEMBER

The HSC Board of Directors will select two (2) HSC paid members who have shown leadership as an HSC volunteer and pay for their scholarships to attend the NHF's 59th Annual Meeting. This includes Airfare and hotel accomodations.  Start preparing your application now. Applications are due not later than June 15, 2007 and need to answer the following two questions:

  1. What have you done in 2008 for HSC and the bleeding disorders community?
  2. What do you plan to do in 2009 for HSC and the bleeding disorders community?

(Note: Answers should also include your actions in the role of leadership within the community.)

Educational Scholarship Instructions:

(Must be Postmarked by June 15th of Each Calendar Year)

Dear Hemophilia Society members,

The goal of the HSC’s scholarship committee is to create the opportunity for success in the post-secondary future of the Society’s members. Two scholarships are currently managed by this committee, and interested members should contact the HSC office to request the applications or download the application below. The Mark Reames Memorial Scholarship offers two (2) $1,000 awards . Secondly, the Mike Conley Memorial Scholarship offers two (2) $1000 awards to any Colorado resident who has a bleeding disorder. Applicants can apply for both scholarships; likewise, previous recipients may reapply to be considered for selection. The scholarships may be used at in-state or out-of-state post-secondary schools, which includes 2- and 4-year schools, vocational/technical schools and graduate school.

In order to insure the timeliness of the awards and to encourage up-to-date applications, Applications are required to be postmarked by May 1 of each year.

Applicants need to keep in mind that the selection panels are not personally familiar with the individual applicants nor with the specifics of hemophilia. Commmittee members for the Mark Reames Selection may or may not be personally impacted by hemophilia, although the Mike Conley Memorial Scholarship includes a Conley family member who also works at Metropolitan State College.

The criteria for the selection process includes the following:

Eligibility of applicant
Demonstrated scholastic ability and intellectual promise (Mike Conley Scholarship offers preference for Metropolitan State College and the University of Colorado at Denver)
Letters of recommendation (within the past year)
Personal essay
Community involvement, with preference given to involvement with the Hemophilia community
Financial need
Incomplete applications will not be reviewed.
